I am looking to create some questionnaires/intakes forms thta will be hosted on wordpress only visible for the people who has the link (whihc i would send via email)
I will need 5 forms.
The important things I need is:
-when selecting a specific name in a drop down menu that an email get sends to the email associated to that name. -the email send to the specific person could be on text or pdf, and will need the questions and then the answers -some fields will need to be compulsory -info does not need to be stored on the website, so no need to keep on data base etc etc -info would need to be sent securely over the internet -happy to use a plugging or a custom made coding -will need to be easy to change to add other names and emails for future changes
