Please describe how your organisation’s infrastructure and setup will enable you to deliver Home Care Services to Individuals within Leicestershire. Please ensure your answer considers the following: • Confirmation of your main office location and any satellite offices you will have to deliver this contract • How your location will ensure seamless delivery of care • How staff will be supported e.g., with access to equipment/PPE, supervisions, team meetings and training • How you will address and manage the urban/rural challenges within the county Taking into account our Outcomes Framework outlined within the Service Specification, what outcomes measuring tools do you intend to use to demonstrate delivery of these? Please share an example to support this where possible Council will take a more outcomes-based approach to performance management of the Services. There will be more focus on the long-term positive changes to enhance Individuals’ independence and quality of life through utilising the Council’s Outcomes Framework (Appendix 1 – Outcomes Framework). Some performance monitoring through KPIs (Appendix 2 – Key Performance Indicators) will still be undertaken. The Provider will be responsible for ensuring they meet the requirements outlined in Appendix 1 – Outcomes Framework and Appendix 2 – Key Performance Indicators. The Council will also collate and monitor information using its own systems to monitor the success of the DPV and to support strategic market management. Any concerns identified through this activity will be raised with the Provider as necessary. The Provider needs to set outcomes with the Individual and then work with them to identify progress against these. The Provider will need to establish a tool for measuring these outcomes. It is expected that any tool used will be adaptable to suit different care groups. Outcomes will be personalised, and ways of measuring achievement of outcomes will be accessible to people on different levels including easy read, pictorial etc. The Provider must seek the views of the Individuals and families/carers using their service, to demonstrate user satisfaction and validate the outcomes achieved. This should be presented in the form of “I” statements linked to the Outcomes Framework (Appendix 1). Information on Individuals’ progress (in either direction) against outcomes should be supplied by The Provider to the Council through contract monitoring meetings at a frequency and on a format agreed, as outlined in Appendix 6 – Quality Monitoring Framework for Commissioned Providers. It is expected that the Provider will work with the Council to ensure continuous improvement in the Service including making recommendations for areas of improvement and innovation in service delivery to meet the stated outcomes.
