As an example: I require a universal operative report template that is tailored to my needs. The template should follow a standard operative report format but exclude all non-essential fields. It should begin with a preoperative diagnosis section that allows for multiple selections. The selected diagnosis(es) will limit the options available in the next two fields, which are postoperative diagnosis and procedure. Once the procedures are chosen, the template should present a list of multiple steps to complete the procedure.
